| Description: | The Patient Flow Coordinator ensures the smooth entry of patients into the care continuum by enabling all necessary preparations for a patient’s care including obtaining all necessary documents related to the admission process. The Patient Flow Coordinator works with other members of the medical care team in coordinating the admission process for the patient. Priority is placed on direct admits, ER admissions, post-op admissions, and discharges. The Patient Flow Coordinator is directed by the Nursing Care Supervisor.
This is a weekend option, day-shift position that will work 24 hours per week on Saturdays and Sundays.

| Duties: | Admission Duties
Assesses patient status and facilitates the efficient movement of patients through the hospital system.
Completes the admission packet on direct admits, ER admissions, post-op admissions and whenever possible, assists or completes discharges.
Works in an efficient, quality-driven manner to provide the highest level of care to newly admitted patients.
Works to obtain information necessary to complete the admission assessment, physical assessment, and begins completion of the plan of care.
Obtains all necessary consents, as needed.
Completes initial documentation on the chart.
Communicates information obtained to the primary care giver to allow a smooth transition.
When and if time permits, the Patient Flow Coordinator will start IV’s and initial doses of medications.
Utilizes the Nursing Process
Obtains accurate nursing history and documents on appropriate patient records.
Assesses, documents, and reports physiological and behavioral changes of the patient.
Formulates an effective, individualized nursing plan of care.
Initiates discharge planning upon admission and coordinates movement toward these plans and goals throughout the admission.
Implements prescribed treatment and nursing plan of care.
Evaluates and revises plan of care as needed.
Implements hospital and nursing guidelines and procedures in clinical practice.
Provides Patient, Family, Staff and Student Education
Identifies present and anticipates future learning needs of patient, family, and staff.
Provides information on health maintenance.
Provides information on the disease process, medications, etc.
Provides counseling and support to patients, family members, and significant others.
Provides information on illness prevention.
Assists in developing meaningful goals for patients, family members, and significant others.
Validates patient comprehension (i.e. return demonstration, verbalization of understanding, etc.)
Assists with co-worker and student orientation and education.
Practices Clinical Competence
Demonstrates competence in basic nursing skills.
Practices proficiency in technical and special skills relevant to area of clinical practice.
Maintains established safety, environmental, and infection control standards and takes appropriate corrective action for identified infractions.
Transcribes and implements physician orders, seeking clarification if needed.
Answers call lights.

Minimum Position Qualifications
Graduate of an accredited school of nursing. BSN required, MSN preferred.
3-5 years of experience as a registered nurse in an acute care setting
Must hold current Indiana nursing licensure. CPR certification required.
