Description:The Occupational Therapist completes evaluation, treatment planning, and direct treatment for physician referred patients. Assesses, plans, organizes, and participates in rehabilitative programs that help restore vocational, homemaking, and daily living skills, as well as general independence, to disabled persons.
Duties: Provides Daily Patient Assessment, Determines Plan of Care, and Provides Appropriate Related Treatment
  • Demonstrates efficient organization and time management skills to complete daily census tasks.
  • Writing is concise, professional, and easily read.
  • Assists physician and related staff, patient, and family with discharge planning.
  • Patient Care reflects appropriate integration of department goals and objectives.

    Demonstrates Responsibility for Skill Development
  • Responsible to staff meetings, care plan meetings, and peer inservices.
  • Participates in appropriate continued education and presents follow-up in-servicing to fellow staff.
  • Utilizes available resources and skilled peers to assist in thorough patient evaluation and planning as appropriate.

    Responds with Positive Teamwork to Direction and Department Planning Strategies
  • Flexes to assigned area of patient census need consistent with competency.
  • Participates in special projects and committees.
  • Participates in program development and quality assurance measures.

    Home Healthcare/Hospice Job Functions
  • Performs Initial and ongoing assessment to determine level of functioning, including OASIS assessments at appropriate time points.
  • Assess the need for other services and refer as necessary.
  • Develops and revises the plan of care in consultation with the physician and other care-team members based on evaluation and medical history and revise as needed.
  • Provides therapeutic treatments and agrrange/provide equipment required to increase the patient’s function and independence.
  • Educate, advise and consult with the patient/family and other agency personnel as applicable.
  • Prepares clinical and progress notes including therapy interventions, initial and ongoing assessment and evaluation of patient/family response to therapy, problem identification and goal settting.
  • Treatment plan in consultation with the physician and IDT/IDG to achieve goals. (Hospice)
  • Provide treatments within the professional standards of practice and state licensure requirements.
  • Evaluates outcomes of care.
  • Consult with IDT/IDG members. (Hospice)
  • Timely recording of all assessment and evaluatation data, treatments and patient’s response to therapy intervention.
  • Supervises Therapy Assistants and Home Health Aides as appropriate.
  • Participates in in-service programs.
  • Plans for discharge, coordinating discharge planning with other team members.
  • Assesses pateint’s home and equipment for safety hazards and architectural barriers.
  • Performs exercises to improve functional activities of daily living.
  • Establishes household management routines with patient and family input.
  • Coordinates plan with physical therapy, if ordered, and avoids duplication of service.
  • Qualifications:
  • Successful completion of fieldwork experience or job experience in related area.
  • The ability to exert maximum muscle force to lift, push, pull, or carry objects.
  • Knowledge of information and techniques needed to rehabilitate physical and mental ailments and to provide career guidance including alternative treatments, rehabilitation equipment and its proper use, and methods to evaluate treatment effects.
  • Home Healthcare/Hospice: Sensitive to issues of loss and grief.

    Minimum Position Qualifications
  • Bachelor’s degree required from an accredited School of Occupational Therapy
  • Certification required from health Professions Licensing Bureau of Indiana (temporary license acceptable).
  • Certification required from National Board for Certification in Occupational Therapy (NBCOT) (registry eligible status acceptable).
