| Description: | The Clinical Social Worker provides psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers, providing patient education and counseling, and making necessary referrals for other social services.

| Duties: | Clinical Social Services include
Works collaboratively with Multidisciplinary Team of physicians, nurses, occupational therapists, and other members of the health care team in meeting patient needs and developing individualized treatment plans.
Provides individual, marital, family, or group therapy for psychiatric patients.
Mobilizes community resources, agencies, and outpatient treatment for follow-up care for patients.
Serves as on-call contact person on a rotation basis, for acute hospital crisis intervention.
Provides consultation and education to other members of the health care team in understanding the significant social and emotional factors related to the illness or disability.
Identifying and utilizing appropriate community resources and assessing patient/family ability to access them
Coordinates social work intervention services through the case manager or designee.
Identifies present and anticipates future learning needs of patient, family, and staff.
Provides information on health maintenance.
Provides information on the disease process, medications, etc.
Provides counseling and support to patients, family members, and significant others.
Provides information on illness prevention.
Assists in developing meaningful goals for patients, family members, and significant others.
Assists patients/families in developing safe and appropriate discharge plan
Practices Clinical Competence
Practices proficiency in technical and special skills relevant to area of clinical practice.
Maintains established safety, environmental, and infection control standards and takes appropriate corrective action for identified infractions.
Transcribes and implements physician orders, paperwork for nursing home, and seeks clarification if needed.
Develop relationships with community agencies to expand knowledge of admission criteria, accessibility, and referral process.
Arranges treatment for patient in need of acute in-patient psychiatric care.

| Qualifications: | Ability to accept physician orders as related to their scope of arranging for continuum of care.
Is knowledgeable of community resources and refers appropriately.
Must be able to assess, interpret, and appropriately react to key data about a patient’s status.
Minimum Position Qualifications
High School Diploma or GED.
Masters prepared Social Worker
Current LCSW preferred.
CPR certification required.
Social Worker experience preferred.
