| Description: | The Patient Care Coordinator (PCC) is a Registered Nurse that fulfills all aspects of the RN job description. In addition, is responsible for overseeing other duties not directly related to patient care. |
| Duties: | Utilizes the Nursing Process
Obtains accurate nursing history and documents on appropriate patient records.
Assesses, documents, and reports physiological and behavioral changes of the patient.
Formulates an effective, individualized nursing plan of care.
Provides Patient, Family, Staff and Student Education
Identifies present and anticipates future learning needs of patient, family, and staff.
Provides information on health maintenance.
Provides information on the disease process, medications, etc.
Provides counseling and support to patients, family members, and significant others.
Provides information on illness prevention.
Assists in developing meaningful goals for patients, family members, and significant others.
Validates patient comprehension (i.e. return demonstration, verbalization of understanding, etc.)
Assists with co-worker and student orientation and education.
Practices Clinical Competence
Demonstrates competence in basic nursing skills.
Practices proficiency in technical and special skills relevant to area of clinical practice.
Maintains established safety, environmental, and infection control standards and takes appropriate corrective action for identified infractions.
Transcribes and implements physician orders, seeking clarification if needed.
Supplies
Responsible for ordering clinical supplies for the office.
Maintains an accurate inventory.
Maintains a good relationship with vendors.
Staff Schedules
Responsible for scheduling clinical and clerical personnel for Nephrology office
Handles coverage issues for illnesses of staff.
Responsible for approving time off and handling coverage issues.
Personnel Management
Functions as a resource person for intradepartmental and interdepartmental issues.
Provides input for clinical and clerical staff performance evaluations.
Involved in the interview and hiring process of new clerical and clinical staff.
Coordinates orientation process for new employees.
Conducts monthly staff meetings to communicate departmental changes and/or issues.

| Qualifications: | Knowledge of nursing care principles, procedures, and practices.
Knowledge of pharmaceuticals, expected beneficial effects, potential side effects and be skilled in safe administration practices.
Ability to recognize adverse patient signs and symptoms and to react appropriately.
Must be able to identify and provide for age-specific patient needs.
Must be able to assess, interpret, and appropriately react to key data about a patient’s status.
Skill in operating job related medical equipment.
Minimum Position Qualifications
Graduate of an accredited school of nursing.
Previous experience in a physician office setting preferred. Previous acute care experience required.
Must hold current Indiana nursing licensure. CPR certification required.
