| Description: | The Speech Pathologist assesses and treats patients with speech, language, and swallowing disorders under the professional standards of practice and state licensure requirements. In Home Healthcare/Hospice, reports to the Regional Director. |
| Duties: | Professional Responsibilities Participates in conferences and training related to the profession or patient care. Makes patient referrals to other professionals as appropriate.
Documents and Records Information Completes daily billing sheets. Completes daily patient follow-up sheets with attached orders for inpatients.
Other Departmental Responsibilities Participates in performance improvement projects to develop or improve procedures/treatments. Participates in regularly scheduled departmental meetings and tasks.
Home Healthcare/Hospice Job Functions Initial and ongoing assessment to identify the client’s level of functioning and ability to communicate, including OASIS assessments at appropriate time points. Performs speech, language and swallowing evaluations to collect information on type and degree of impairment. Testing and recommending mechanisms which focus on such interventions as alternative methods of communication/speech and swallowing exercises to help with nutrition. Ongoing assessment, evaluation and documentation of patient’s level of functioning and hearing in response to therapy. Develops plan of care in collaboration with the physician-based upon evaluation and medical history. Prepares clinical and progress notes. Consults and coordinates services with other disciplines as needed. Instructs patient in techniques to improve speech, language and swallowing deficits. Educates and consults with patient/family and other organizational personnel involved in patient care. Plants and coordinates discharge planning with patient and other team members. Makes patient referrals to other professionals as appropriate. Documents success or failure in meeting established goals. Setting of goals for patients based on the needs of the hospice patient/family. (Hospice) Communicate information to IDT/IDG members. Participates in performance improvement projects to develop or improve procedures/treatments. |
| Qualifications: | Competent in the evaluation and treatment of speech, language, and swallowing disorders. Competent in counseling patients, family members and other professionals in speech, language, and swallowing disorders. Competent in communicating results of evaluations and current patient status to other professionals. Competent in completing patient evaluation reports and progress notes in Home Healthcare/Hospice. Sensitive to issues of loss and grief.
Minimum Position Qualifications Master’s degree in Speech Language Pathology required Must have Indiana licensure or be eligible for licensure. Must pass National Praxis Exam and have or be eligible for ASHA CCC’s in Speech Pathology. CPR certification required |
