The Patient Care Coordinator (PCC) is the Registered Nurse focused on and directing the unit's patient care delivery, customer satisfaction, quality sustainability, and staff development efforts. 


Patient Care Navigation Responsibilities:

  • Collaborates with nursing supervisor for bed placement and staffing.
  • Demonstrates understanding and applies staffing guidelines and PCM Auditor
  • Demonstrates understanding and applies Synergy Model and Acuity classification system.
  • Assumes the expert role in Synergy's patient classification system after staff members' initial training. 
  • Promotes best practice and system process improvements as an extension of the Clinical Nurse Specialist.
  • Understands collaboration with CNS and the LEAP model.
  • Assures documentation supports patient acuity, which supports charging system and productivity summary.  Documentation supports quality. 
  • Understands revenue cycle and proper classifications and actions taken as appropriate to classifications; prevents delays in care.
  • Is an integral part of the operational metrics by demonstrating a thorough understanding of throughput and which metrics require most follow up from the PCC.
  • Accurately audits documentation to ensure it is correct. 
  • Performs concurrent review for proper classifications.
  • Accurately identifies patient acuity for care assignments and patient outcomes. 
  • Understands CMS, HFAP, unit goals and expectations.
  • Understands evidence based order sets (EBOS) and operationalizes by using key phrases to guide physicians to use.  Oversees that EBOS are placed on appropriate charts.
  • Assures documentation supports patient acuity, assuring quality, throughput, and maximum reimbursement. 
  • Evaluates and monitors the nursing staff for expected quality outcomes:  no falls, no skin breakdown, no med errors, etc. 
  • Knows protocols for "presence on admission" wounds, restraints, falls, and other key medical processes. 
  • Evaluates how changes in processes affect outcomes and takes responsibility and ownership of situations that can be controlled.  For example, how new shower design affects fall rates, and implement improved processes to reduce any identified risks.
  • Assumes responsibility for patient throughput and timely discharge in conjunction with bedside nurse and Case Management.
  • Advocates interdisciplinary care conferences and assists as needed.
  • Facilitates as needed with timely hospitalist rounding and physician intervention.
  • Coordinates patient assignments utilizing the Synergy model of practice. 
  • Demonstrates knowledge of standards of care and act as a clinical resource to interpret and implement standards of patient care. 
  • Triages changes in patient acuity and assists in the continuity and timeliness of patient care.  Acts as an extender to provide clinical support to staff, physicians, etc. 
  • Performs patient care when needed to keep schedules on track.  Examples:  starts IVs, assists with a procedure, etc.
  • Demonstrates strong understanding of Milliman and length of stay.
  • Promotes patient, family, and physician satisfaction.
  • Assures AIDET or other initiatives are implemented by staff.  Reminds staff of phone etiquette.
  • Provides oversight of coordinated care by nursing and non-nursing care providers.
  • Focuses on staff compliance/follow through with safety and quality initiatives. 
  • Demonstrates knowledge of patient needs and assigned caregivers' personal and professional traits for positive interaction and goal-oriented care.
  • Performs objective investigation of issues on the unit, such as follow up to patient complaints and/or care incidents, and prompt resolution/follow up with staff, patients/families, and physicians.
  • Understanding, supporting, mentoring/modeling/managing, coaching, and rewarding of appropriate staff interventions and interactions. 
  • Tracks and trends satisfaction information and has defined role in making improvements. 
  • Performs observation rounds of staff compliance with handwashing, AIDET, and service excellence initiatives for the organization.  

Personnel Management:

Shift Management:

  • Demonstrates effective conflict identification and resolution skills.
  • Provides written documentation in PCM Auditor or using other electronic staffing documentation when staffing exceeds budgeted target.
  • Manages staffing for assigned shift and makes adjustments as needed for oncoming shifts, minimally up to 24 hour period.  Understands productivity reports and makes decisions for positive impact for patients and organization.

Unit Management:

  • Serves as the nurse in charge for the shift.
  • Utilizes knowledge of federal, state, and accreditation standards to assure that unit is survey-ready at all times.
  • Responsible for the enforcement of policies and procedures that adhere to unit-specific as well as hospital-wide Human Resources policies and procedures/standards.
  • Has knowledge of corrective action process and appropriately relays significant clinical or behavioral issues to Department Director and documents discussions with staff.
  • Follows up with TB testing, education CBTs, and other compliance issues.
  • Supports and Provides Education for Staff and Students
  • Coordinates student and patient activities with clinical instructors to create an environment that enhances student and patient experiences. 
  • Understands student documentation limitations and co-signatures.
  • Assures that licensed staff maintain responsibility for patient care and outcomes.
  • Promotes a mentorship attitude among staff and students.
  • Promotes staff professional growth as a part of daily routine.  Mentors and coaches staff at the bedside in their development from Novice to Expert nurses, utilizing competencies as defined by Staff Education, to evaluate nurse skill levels and assist with sustaining their expertise.
  • Maintains balance between competency-building experiences and patient throughput. Supports less experienced caregivers to assure timely progress toward discharge goals.
  • Employs fair, equitable, objective evaluation of personnel.  Formally and informally monitors staff to sustain performance, competence, quality, and identify education needs.  Responsible to provide immediate feedback to staff regarding clinical performance and behaviors and documents for inclusion in personnel file. 
  • Documents individual education needs and assists in developing plans to address issues in conjunction with Staff Education and unit Education Coordinator.  Documents follow up and resolution. 
  • Graduate from an accredited school of nursing.  BSN required, or actively pursuing/willing to pursue BSN and completing within 3 to 5 years after beginning.  Must be prepared to begin BSN program within one year of starting in the PCC position.
  • Two years of relevant clinical nursing experience.
  • Preferred: Supervisory or charge nurse experience
  • Must hold current Indiana nursing licensure.   CPR certification required.
  • Must demonstrate leadership abilities and effective conflict resolution skills.
  • Knowledge of pharmaceuticals, expected beneficial effects, potential side effects and be skilled in safe administration practices.
  • Knowledge of nursing care principles, procedures, and practices.
  • Ability to recognize adverse patient signs and symptoms and to react appropriately.
  • Must be able to assess, interpret, and appropriately react to key data about a patient's status.
  • Must be able to identify and provide for age-specific patient needs.
  • Skill in operating job related medical equipment.
  • Must maintain effective, professional interpersonal working relationships.
  • Must be able to successfully interface across departments and assist in the development of strong interdepartmental relationships.
  • Must have demonstrated ability to create strong physician partnerships to move change forward.
  • Must have strong analytical skills along with appropriate computer skills.
